Surveys 
These are the Easy Read and standard versions of the Evidence Uncertainty surveys for service users, families and carers, and service providers. 
These are the Easy Read and standard versions of the Shortlisting surveys for service users, families and carers, and service providers.  In the Easy Read version, the demographic questions and shortlisting section were provided as separate documents.
Promotional videos
Service users and service providers created a video to explain what the project was about and why they chose to get involved. This was used to promote the Evidence Uncertainty survey, and was distributed along with the survey links and option to request hard copies.

 

 

A video was developed to promote the Shortlisting survey, featuring service users and service providers explaining the importance of participation. It was distributed alongside the survey links, with an option to request hard copies.
